2019-02-23 · Point shear-wave elastographic (pSWE) techniques use acoustic radiation force impulse (ARFI) to stimulate the liver tissue and to generate shear waves that propagate into the liver. The shear-wave velocity (SWV) increases with the severity of fibrosis. The first type of pSWE was Virtual Touch Quantification (VTQ) developed by Siemens, followed by ElastPQ by Philips, and nowadays pSWE is Philips shear wave elastography simplifies liver assessment, making obtaining liver stiffness measurements fast and easy. This non-invasive, reproducible, and easily performed method of assessing liver tissue stiffness may help reduce, or even avoid, the need for conventional liver biopsies 1.

Elastography is divided into strain elastography and SWE. Shear wave elastography is an imaging technique which quantifies liver stiffness, by measuring the speed of sheer waves in the liver tissue. By measuring the speed of the ultrasound in the liver tissue, the underlying stiffness of the liver is quantified. A low speed corresponds to a soft liver, while a high speed indicates stiffer tissue. Methods: Shear modulus of the biceps brachii was determined using ultrasound shear‐wave elastography during static elbow flexion (up to 60% of maximal contraction) in healthy young adults.

Ultrasound shear wave elastography of the bowel wall was performed by two methods (Virtual Touch Quantification [VTQ] and Virtual Touch-IQ [VT-IQ]; Siemens Medical Solutions USA, Inc, Mountain View, CA). Eighteen short-axis shear wave speed measurements were acquired from each specimen: 3 from the 9-, 12-, and 3-o’clock locations for each method.

In shear wave velocity measurements, shear waves are generated by applying mechanical vibration or pressure or by applying acoustic pressure. Shear waves could be better for ultrasound imaging, but they have been harder to produce. × Ultrasound imaging could produce far better pictures with a different kind of sound wave. These higher-performing sound waves, so-called shear waves, are usually hard to generate, but now researchers have demonstrated a simple way to produce them.

Shear Wave Generation with EMATs. Shear waves have an inherent polarization direction depending on how they are generated. Pictured below are horizontally polarized shear waves propagating along the length of a plate. Electromagnetic-acoustic transducers (EMAT) designed to generate shear waves actually vibrate the atoms within the material

The multifidus muscle is Shear-wave ultrasound elastography exploits the acoustic radiation force induced by ultrasound beams to displace tissue and generate shear waves. Acoustic pulses can be focused at different depths in the tissue at supersonic speed and are enhanced by forming a Mach cone, which increases shear-wave propagation.
